Just to add a bit to this, this seems to only occur when the destination folder is set to something that's not workspace, which is probably 90% of the time anyway ;)

This issue is closed.
The upload field used to delete a file from disk when the file is "removed" from an entry. That is broken now.
(Using latest integration code.)
