But fields[issue-number] is correctly set with a value of 1 in the form. Perhaps it’s a custom event that is unset-ing the variable somewhere.

Is fields[issue-number] the only field that the validator complains about? I’ll gladly help out with testing if needed to fix this issue.

Should be fixed. The issue number is pulled in the custom event, based on existing issues. If there are no existing issues, the SQL was returning NULL instead of 1.
This issue is closed.
The create new issue form for extensions doesn’t work. It simply returns me to the form itself.
