What's Error 256 on log??

I’ve got a form to send a file (.doc, .docx, .pdf), but, sometimes, the form doesn’t send it.
I get the log file, and there’s the message **2010/09/20 17:42:31 > ERROR: 256 - **, all over the log.
Can someone help me?
